I will send you two intake forms as an email attachment. One is for your teenager to complete, and one is for the parents to complete.
I can usually schedule an appointment for your adolescent within 30 days, sometimes sooner. lf you have a true emergency, I will see you the same day that you call.

If you are at a local hospital with your adolescent, and you need a consultation for a psychiatric admission, I am also available.
All major insurance plans are accepted. Payment by credit card, cash, check, direct deposit, or a payment plan can be arranged. Each session is 2 hours, at a total cost of $250.

One parent must be on the premises at all times. There are no exceptions to this rule.
